Our nutrition expert will then provide you with the recommendations you need to see results, and a convenient 3-day meal plan to help you succeed.
How does this work?You will receive your detailed Nutrition Assessment in a report form AND your convenient 3-day meal plan in an easy to read format within 72 hours from the time we receive the required forms and payment. We will follow up with you to schedule a 45 minute phone conversation to review your results and/or discuss any questions you may have. This option also includes a free month of nutrition coaching.

What do I need to do?

• Fill out and submit the MMF.com Nutrition questionaire.
• Fill out the type and amount of food you eat each day for three days using the MMF.com Daily Food Log.
• Sign and fax us the MMF.com nutrition waver and liability form
